Before Consolidate Health, I spent far too much time navigating fragmented patient records. Now, with AI-driven data aggregation and NLP, I see the full clinical picture—labs, pathology, imaging, and referrals—within seconds. It’s like having a virtual assistant that thinks like a clinician. It’s transformed how I prepare for appointments, allowing me to focus on decisions that improve outcomes, not paperwork. In value-based oncology, that’s a game-changer.

The Liver Institute at Methodist, Dallas

Imaging Modality: CT Head without Contrast
Indication: Evaluation of persistent headaches and dizziness.

Summary
The CT scan of the head shows mild diffuse cerebral atrophy, which is consistent with the patient’s age. There is no evidence of acute intracranial hemorrhage, mass effect, or midline shift. The ventricles appear normal in size and configuration. There are severe periventricular white matter hypodensities, likely indicating chronic ischemic changes. The sinuses show mild mucosal thickening in the maxillary regions, without air-fluid levels or significant sinus disease. The orbits are normal with no evidence of proptosis or abnormal masses. Additionally, there are no fractures or destructive bone lesions noted in the skull.

The scan reveals mild age-related cerebral atrophy and no acute intracranial abnormalities. There are also mild chronic ischemic white matter changes.

Assessment & Plan

Apr 14, 2024

Further evaluation will include PSA testing for prostate cancer risk assessment and monitoring treatment response, along with uroflowmetry and PVR measurement to assess urinary flow and residual volume. Regular monitoring of renal function and PSA levels will be conducted to ensure continued management effectiveness.

Follow-up in 4-6 weeks is scheduled to reassess symptoms, perform repeat studies as needed, and consider referral to urology for procedural intervention if symptoms persist.
